Job summary

Serenity Hospice Care in New Jersey seeks a compassionate Certified Home Health Aide (CHHA) to provide patient-centered hospice care in the Mercer County area, focusing on evenings 3:00 PM–11:00 PM Monday–Friday.

The role involves assisting with ADLs, following care plans, collaborating with the interdisciplinary team, and communicating with patients and families; NJ CHHA license required; hospice experience is a plus.

Qualifications

We are currently seeking compassionate and dedicated Certified Home Health Aides (CHHAs) to join our growing team in the Mercer County area. This position focuses on delivering high-quality, dignified care to patients during the evening shift Monday-Friday (3:00 PM – 11:00 PM).

Responsibilities

• Provide compassionate, hands-on care to hospice patients as needed

• Assist patients with activities of daily living (ADLs)

• Follow individualized care plans to ensure the highest level of comfort and support

• Collaborate with the interdisciplinary hospice care team

• Communicate effectively with patients and their families

Requirements

• Valid Certified Home Health Aide (CHHA) license in the state of New Jersey

• Prior hospice experience is a plus, but not required

• Reliable transportation and dependability

• Strong communication skills and a compassionate approach to care

Evening Shift: 3:00 PM – 11:00 PM
