Job summary

21st Century Home Health Services (21HHS) in Grass Valley, CA is seeking a Full-time Certified Home Health Aide (CHHA) to provide in-home patient care under RN supervision.

You will perform personal care, assist with daily activities, monitor conditions, and document in electronic charting while collaborating with the interdisciplinary team. A valid CHHA CA certification, CPR/BLS, and CA driver's license are required.

Qualifications

• CA CHHA certification is mandatory for the role.
• Current BLS/CPR certification is required.
• Valid CA driver’s license is required.
• Home health care experience preferred; high-acuity patients a plus.

Responsibilities

• Provide personal care including baths, grooming, and hygiene.
• Assist with dressing, mobility, and daily activities.
• Maintain skin care and prevention of breakdown.

Full time Certified Home Health Aide (CHHA)

Territory: Grass Valley

Under the direct supervision of an RN Case Manager, you will provide patient care according to the Plan of Care.

• Provide personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen
• Assist patients with dressing and undressing
• Provide skin care to prevent breakdown
• Assist the patient with toileting activities
• Keep patient’s living area clean and orderly, as appropriate
• Assist in feeding the patient, if necessary.
• Perform range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
• Assist patient in the self-administration of medication.
• Meet safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
• Report on patient’s condition and significant changes to the assigned nurse.
