Job summary

Premier Infusion and Healthcare Services, Inc. is looking for a Certified Home Health Aide (CHHA) to provide personal care and supportive services to patients in a home setting. The ideal candidate will have a CHHA certification and previous experience in a caregiving role.

Responsibilities include assisting with daily living activities, meal preparation, and monitoring patient conditions, all under the supervision of a clinical supervisor. Strong communication and organizational skills are essential for this role.

Qualifications

• Current experience providing patient care in a home health, healthcare, or caregiving setting preferred.
• One (1) year of Home Health experience preferred.
• Previous administrative or clerical experience in a healthcare setting preferred.
• One (1) year of experience with EMR systems preferred.
• Strong verbal and written communication, organizational, and documentation skills required.

• Assisting in feeding the patient, if necessary.
• Taking and recording patient vitals when ordered.
• Reporting on patient’s condition to the assigned nurse.

Description of Responsibilities

The Certified Home Health Aide (CHHA) is responsible for delivering personal care and assistance to patients within the home environment under the supervision of a Registered Nurse or Therapist. The role includes assisting with activities of daily living, monitoring patient condition, maintaining a safe support environment, and providing administrative support during periods of low census.

Responsibilities

1. Providing hands‑on personal care including but not limited to:
   • Baths
   • Oral hygiene
   • Assisting patients with dressing and undressing
   • Skin care to prevent breakdown
   • Assisting the patient with toileting activities
   • Keeping patient’s living area clean and orderly, as appropriate
2. Planning and preparing nutritious meals.
3. Assisting in feeding the patient, if necessary.
4. Taking and recording oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure when ordered.
5. Performing range‑of‑motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).
6. Reporting on patient’s condition and significant changes to the assigned nurse.

Minimum Qualifications

• Current experience providing patient care in a home health, healthcare, or caregiving setting preferred.
• One (1) year of Home Health experience preferred.
• Previous administrative or clerical experience in a healthcare setting preferred.
• One (1) year of experience with EMR systems preferred; CareTend, KanTime, or CPR+ experience preferred.
• Strong verbal and written communication, organizational, and documentation skills required.

Education and/or Experience

• Current Certified Home Health Aide (CHHA) certification in the State of California required.
• Valid California driver’s license, reliable transportation, and current automobile insurance required.
