Job summary

Newport Hospice is seeking a Certified Home Health Aide to provide one-on-one care in patient homes, following a personalized care plan. You will build strong relationships with patients, maintain detailed notes, and communicate observations to a supervisor. A compassionate, organized approach and strong communication are essential.

Prior Home Health or Hospice experience is a plus. A valid driver’s license and CPR certification are required to ensure safe and reliable care delivery in a

• Must have High School Diploma or GED.
• Driver's license required.
• CPR certification required.
• One year of prior professional experience preferred but not required.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles.
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care.
• Maintain records of patient care, condition, progress, or problems to report to the supervisor.
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert.
• Engage patients in exercises or other activities.
