We are looking for a Full-time Certified Diabetes Educator/RD to promote healthy eating habits through individual nutritional assessment, diagnostic therapy, and counseling services, to educate patients and their families with or at risk for diabetes on self-management. Collaborating with multidisciplinary teams, you will perform in-depth motivational interviews to identify prospective and fully-fledge diabetes; develop a treatment program based on evidence-based procedures and regulatory requirements and monitor adherence. You will also train and educate nursing staff; attend meetings, in-service training, and outreach activities; and participate in quality assurance/improvement monitoring and reporting initiatives.
