Job summary

Baylor Scott & White Health in the Greater Austin, TX area is seeking a Certified Diabetes Educator to deliver structured diabetes self-management education to patients at risk and with diabetes. The role involves direct patient education, program development, and collaboration with the care team to improve outcomes.

Onsite position with clinics in the Austin metro area; preference for CDE certification within 12 months and experience with diabetes technology and education standards.

Qualifications

• Bachelor's degree in a relevant field.
• Less than 1 year of experience.
• Certified Diabetes Educator preferred within 12 months of hire.
• BLS required or within 30 days of hire.
• RD/RN/RPH certifications preferred (as applicable).

Responsibilities

• Plans, directs, and provides individualized self-management education for patients and families.
• Develops and conducts educational programs and in-service training.
• Serves as a resource in diabetes care and education.
• Optimizes care delivery, patient/provider experience, quality, and costs.
• Applies teaching principles to facilitate self-management.
• Tailors interventions to individual needs.
• Communicates with patients and healthcare team in a timely manner.
• Participates in quality improvement activities.
• Engages in professional development to maintain diabetes qualifications (15 hours annually).

Job Summary

100% onsite in the Greater Austin, TX area

• Outreach clinics are located in Hutto, Taylor, Cedar Park, and Pflugerville (clinic frequency TBD).
• The Certified Diabetes Educator works with people at risk and with diabetes to achieve behavior change goals which lead to better clinical outcomes and improved health status.

Essential Functions Of The Role

• Plans, directs, and provides individualized self-management education, training and help to patients and families in a culturally-competent manner across the lifespan.
• Develops and conducts educational programs and in-service training programs.
• Serves as a resource to staff and community as a diabetes care and education specialist.
• Optimize care delivery, patient and provider experience, quality, safety, and costs.
• Applies current principles of teaching and learning and/or behavior change to facilitate self-management skills.
• Tailors interventions to individual self-management needs.
• Communicates information to patients and healthcare team in a timely manner. Serves as a role model, resource, and mentor to other team members.
• Participates in quality improvement activities to help process improvement and clinic/facility/system goals.
• Participates in professional development activities to maintain and improve required qualifications; obtain 15 diabetes-related Continuing Education hours annually.

KEY SUCCESS FACTORS

• Advanced knowledge of pre-diabetes and diabetes care modalities, therapies, terminology and equipment.
• Demonstrates familiarity with pre-diabetes and diabetes pathophysiology, epidemiology, and evidence-based clinical practice.
• Demonstrates familiarity with ADA Education Recognition requirements or National Standards for Diabetes Self-Management Education and Support, if in an ADA Recognized program.
• Must be able to communicate thoughts clearly; both verbally and in writing.
• Ability to effectively use a variety of teaching methods.
• Knowledge of the geographic area, the community and its resources.
• Cultural proficiency related to health practices.
• Committed, dependable, adjustable, original, resourceful and has an excellent desire to help the community.
• Able to interact positively with patients as needed.
• General computer skills, including but not limited to: using required software applications, data entry, information security, electronic medical documentation, hand held scanning and email.
• Certified Diabetes Educator certification strongly preferred within 12 months of hire.
• Knowledge of insulin Pumps and Continuous Glucose Monitoring (CGM) devices strongly preferred.

• Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.
• Registered Dietitians (RD), Registered Nurse (RN), Pharmacist (RPH): Must have one of the following:
• RD - Registered Dietician
• RN - Registered Nurse
• RPH - Pharmacist.
