AbsoluteCare Medical Center & Pharmacy in Cleveland, OH is seeking a Community Health Worker to connect members with resources, address social needs, and enroll them in services. You will visit homes, partner with care teams, and guide enrollment in internal and external programs.

Schedule: Monday through Friday, 8:00 AM to 5:00 PM (1 - hour lunch)
Compensation : $40,000 - $50,000 a year (May increase based on experience)
Active Ohio Community Health Worker certification is required.
This position requires regular travel throughout Cleveland communities to conduct in-person member outreach and engagement. Candidates must possess a valid driver's license, maintain proof of auto insurance, and have access to a reliable personal vehicle. Eligible business mileage will be reimbursed in accordance with company policy.
Community paramedicine experience is a plus, but not required.
The Community Health Worker goes beyond medicine by connecting AbsoluteCare members with resources that help address their social needs and overcome barriers to care. This role fosters trust and builds meaningful relationships with members, meeting them wherever they are, whether at home, in the community, at the hospital, or in an office setting. The Community Health Worker will also reach out to new members, educating them about AbsoluteCare and guiding them through enrollment in both internal and external services. This position operates in a matrixed environment, with dual reporting to the Community Team Care Manager for daily care management and the Engagement Manager/Director for performance evaluation, strategic alignment, and program oversight.
This job operates in the community, in members’ homes, and in the AbsoluteCare clinical center. This role requires a dedicated, quiet workspace with the ability to adhere to HIPAA and other privacy policies. A reliable and high-speed Wi-Fi connection. This role routinely uses general office equipment. It is required that you have and can use your personal vehicle to conduct outreach including member home visits.
