Job description

SALARY RANGE: $53,600-$80,400 (Based on experience)

Position Summary

The Certified Addiction Counselor (CAC) functions as part of the clinical team responsible for delivering rehabilitation services to a group of consumers receiving services from the Community Connections ACT Program under the supervision of the ACT Program director.

Key Responsibilities

• Facilitates rehabilitation, psycho-educational, and support groups.
• Serves as the chosen Rehabilitation Clinician as chosen by the consumer.
• Participates in the development of the ISSP and conduct ongoing periodic reviews, meeting with the consumer, family and/or other network members.
• Completes all documentation necessary for good clinical care and accurate billing. One hundred percent compliance is expected.
• Provides one-to-one counseling as requested by the consumer.
• Participates in training relevant to position and to the maintenance of license.
• Assists with the preparation of the program’s daily lunch.
• Liaison with CSA Clinical Manager.

DESIRED KNOWLEDGE/SKILLS/ABILITIES

POSITION TYPE/EXPECTED HOURS OF WORK

• This is an essential or critical position which will require reporting to a physical work location or making visits to consumers in the community as defined by the business needs of Community Connections, Inc.
