Job summary

The Heights of Atascosa in Pleasanton, TX is seeking a Certified Wellness Director (CNA) who is passionate about making lives better by creating engaging programs for residents and promoting a vibrant, connected community. This role helps deliver The Touchstone Experience where compassion and care come together.

Key duties include planning and leading wellness activities, creating monthly calendars, documenting care in state guidelines, participating in the interdisciplinary team, and supporting

Qualifications

• Current Certified Activity Director certification (required).
• Prior Wellness/Activities Director experience in a SNF preferred.
• Strong organizational, communication, and engagement skills.

Responsibilities

• Plan, schedule, and lead individual and group wellness activities based on resident assessments and interests.
• Create and maintain engaging monthly activity calendars reflecting resident preferences and community events.
• Complete assigned MDS sections accurately and timely.
• Utilize RAPs to determine activity-related care planning needs.
• Develop, document, and update individualized activity care plans (reviewed at least quarterly).
• Document resident and family interactions per state and federal guidelines.
• Serve as an active member of the interdisciplinary team.
• Plan and assist with Residents’ Council meetings.
• Support a positive, energetic environment for residents and team members.
