Job summary

BoldAge PACE centers seek a Center Support Aide to help ensure day-to-day operations run smoothly and safely. You will organize care spaces, assist with meal service and activities, and keep environments clean and welcoming for participants, staff, and visitors.

Responsibilities include escorting participants, restocking supplies, and communicating needs to staff. Prior experience with seniors and a valid CNA/CHHA credential are preferred.

Qualifications

• Two years of experience in hospital, nursing home, or community setting preferred.
• State requirements apply; CNAs/CHHAs may be required depending on location.
• 1 year experience with frail or elderly population preferred; training provided if needed.
• Competencies for Personal Care Specialist will be met before participant care.

Responsibilities

• Assist with daily setup, organization and breakdown of center spaces and meals.
• Escort participants within the center under supervision and aid transfers as needed.
• Support meal service operations, tray setup, delivery, and dining area cleanup.
• Maintain cleanliness and order of shared spaces following infection control policies.
• Restock center supplies such as linens, PPE, and personal care items.
• Monitor safety and report issues to leadership; provide respectful participant interaction.
• Participate in IDT for PCA role as assigned by center leadership.

Center Support Aide

POSITION SUMMARY

The Center Support Aide provides non-clinical support to ensure the PACE center functions smoothly and safely each day. Assists with the setup, organization, and breakdown of personal care spaces, meal service operations, and common areas. Maintains a clean, orderly, and welcoming environment for participants, staff, and visitors. Supports center programming and daily workflows through supply restocking, room preparation, participant escorting within the center, and personal care assistance. Functions as a supportive team member by communicating environmental, medical or operational needs to appropriate staff to promote positive participant experience. Administers personal care assistance when needed to PPTs in Center.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Assist with the daily setup, organization, and breakdown of personal care rooms/center, including arranging tables, chairs, hydration stations, and maintainingsupplies needed for meals and/or personal care.
• Provide non-clinical support during participant activities by escorting participants within the center under staff supervision, including personal careor transfers.
• Assist with meal service operations, including tray setup, table preparation, meal delivery support, and dining area cleanup. Assist withfeeding needs of PPT’s when needed.
• Maintain cleanliness and order of common areas, including activity rooms, dining spaces, hydration stations, and shared equipment areas, closely following the Infection Controlpolicies andprocedures.
• Restock center supplies such as linens, PPE, personal care supplies, PPTclothing, sanitationsupplies, and participant comfort items.
• Support transportation coordination by helping stage participants for departures and arrivals, escorting wheelchairs only.
• Monitor the center environment for safety concerns such as spills, broken equipment, cluttered walkways, or supply shortages and promptly report issues to leadership.
• Foster positive, respectful, and supportive relationships with participants, families, and team members through professional non-clinical interaction.
• Follow all organizational policies, confidentiality standards, and OSHA safety guideline sat all times.
• Perform other non-clinical duties as assigned to support the daily operations of the PACE center.
• Administer personal care such as toileting, showering, personal care, incontinence of care, and colostomy care, as designated within the scope of practice of state certification.
• Maintain required documentation for toileting, showering, laundry, and water temperatures.
• May take assignments to cover home care needs of participants
• Participates in IDT for PCA role as assigned by center leadership

EXPERIENCE AND EDUCATION:

• Two (2) years of experience working in a hospital, nursing home or community-based setting is preferable.
• License/Certifications applicable per State requirements. NJ must have CHHA/CNA
• 1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
• Position specific competencies for the Personal Care Specialist willbe met prior to assuming participant care.

PRE-EMPLOYMENT REQUIREMENTS:

• Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
• Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen, valid driver's license where/if applicable.
