• Initiate admission assessment within 8 hours of admission; teach patient/family.
• Document all patient care per policy and standards.
• Verify patient identity with two identifiers before invasive procedures.
• Follow CDC hand hygiene guidelines and universal precautions.
• Support discharge/transfer planning and timely communication.
