Key Responsibilities
- Providecase management to people who transition from Intermediate Care Facilities intoHome and Community-Based Services.
- Provideeducation to individuals and families regarding Home and Community-BasedServices and available service options.
- Provideinformation concerning rights, options, benefits, services, Person CenteredPlanning goals and expectations.
- Completefield work assignments used to implement Person Centered Support Plans(PCSP), budgets, RFS tasks, Charting the Life Course, and UCANSassessments.
- Coordinateand refer services of community resources, placement providers, collateralagencies, the courts, and/or other community partners.
- Collaborate with other state and local programs, organizations and agencies, and withvarious stakeholders to promote program goals and objectives.
- Provide consultation, make recommendations, give appropriateadvice, and/or assist to facilitate decisions.
Qualifications
Minimum or "day-one" qualifications:
- Hasa Bachelor's Degree with at least 1 year of relevant experience workingdirectly with people with disabilities.
- Hasknowledge of Home and Community Based Services (HCBS) and Settings Rule.
- Has obtained certification as a QIDP (Qualified IntellectualDisability Professional).
Preferred or trainable/on-the-job qualifications:
- Hasstrong interpersonal skills.
- Ispassionate about empowering people with disabilities with working knowledge ofHome and Community-Based Services (HCBS), the Community Transition Waiver andHCBS Settings Rule
- Isan excellent listener who is also very proactive
- Hasknowledge of community resources for people with disabilities and how to accessthose resources
- Hasthe ability to work well with a wide range of people with varying interests
- Canprioritize multiple tasks while maintaining attention to detail
- Is self-motivated with ability to work a flexible schedule
