Job summary

Camino Real Community Mhmr Ctr in Texas is seeking a MCOT Case Manager to provide community-based crisis intervention for individuals with acute behavioral health and substance use crises. You will respond to complex needs, perform crisis assessments, safety planning, referrals, and link clients to ongoing treatment across diverse settings.

The role requires travel across Maverick, Dimmit, and Zavala counties, an on-call rotation, and availability evenings/weekends.

Qualifications

  • Bachelor's degree or RN and QMHP eligibility.
  • Minimum of three years of full-time experience in case management, care coordination, or direct services in human services/behavioral health.
  • Must meet Texas QMHP requirements.
  • Must reside within or respond to Maverick, Dimmit, or Zavala County with crisis response times.
  • Bilingual English/Spanish required.
  • Valid driver's license and ability to travel extensively within the service area.

Responsibilities

  • Conduct crisis assessments and develop safety plans and interventions.
  • Provide community-based crisis intervention services to individuals in acute distress.
  • Coordinate referrals and linkage to ongoing treatment and community resources.
  • Document services and collaborate with healthcare providers, law enforcement, schools, hospitals, and community partners.
  • Respond in person to activated crisis calls within one hour.
  • Extensive local travel; participate in on-call rotations; evenings/weekends; possible overtime.
  • Maintain clinical documentation and data systems; prepare reports and correspondence; participate in trainings and staff meetings.

ISection B - General Description Of Duties And Responsibilities

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other related duties may be required and assigned.


Under the direction of the Crisis Team Lead, the MCOT Case Manager provides community-based crisis intervention services to individuals experiencing acute behavioral health and substance use crises. This position responds to individuals presenting with complex needs, including suicidal ideation, depression, psychosis, substance use disorders, emotional distress, and potential risk of harm to self or others.


Key responsibilities include conducting crisis assessments, developing safety plans and therapeutic interventions, coordinating referrals, facilitating linkage to ongoing treatment and community resources, and supporting individuals and families through crisis stabilization and recovery. The MCOT Case Manager monitors client progress, documents services, and collaborates with healthcare providers, law enforcement, schools, hospitals, and other community partners to ensure continuity of care and safety.


Services are delivered in a variety of settings, including homes, schools, hospitals, jails, emergency departments, and other community locations. Team members must be able to respond in person to activated crisis calls within one hour, regardless of urgency level.


This position requires extensive local travel, participation in an on-call rotation, flexibility to work evenings and weekends, rotating schedules, and occasional overtime to meet community crisis response needs.


Additional responsibilities include maintaining accurate clinical documentation and data systems, preparing reports and correspondence, participating in required training and staff meetings, and ensuring compliance with agency, state, and program standards.

I Section C - Minimum Job Qualifications


  • Bachelor's degree from an accredited college or university in Social Work, Psychology, Sociology, Counseling, Human Services, Nursing, or a related behavioral health field; or be a Registered Nurse (RN).

  • Minimum of three (3) years of full-time experience providing case management, care coordination, or direct services within human services, behavioral health, healthcare, or related setting.

  • Must meet Texas Qualified Mental Health Professional (QMHP) requirements.

  • Must reside within or be able to reliably respond within the designated service area of Maverick, Dimmit, or Zavala County to meet crisis response time requirements.

  • Bilingual proficiency in English and Spanish is required.

  • Valid driver's license, reliable transportation, and ability to travel extensively throughout the service area.


I Section D - Preferred Job Qualifications


  • Experience working in a community mental health center, behavioral health organization, crisis services program, or healthcare setting.

  • Knowledge of Texas mental health laws and procedures related to Emergency Detention Orders, Mental Health Warrants, Orders of Protective Custody, and court-ordered outpatient treatment.

  • Experience working with individuals experiencing serious mental illness, substance use disorders, co-occurring conditions, or behavioral health crises.

  • Knowledge of community resources, systems of care, and service coordination within rural or underserved communities.

  • Training or experience in crisis intervention, suicide prevention, de-escalation techniques, and trauma-informed care.

  • Familiarity with evidence-based practices and recovery-oriented approaches to behavioral healthcare.

  • Experience collaborating with hospitals, schools, law enforcement, emergency medical services, and other community partners.
