Function

The Case Planning Supervisor provides leadership, supervision, and support to the case planning team that delivers services to youth and families in the foster care system. The Supervisor works with the team to adhere to ACS, OCFS, and SCO policies, ensuring safety, permanency, and well‑being of all children and youth.

Responsibilities
  • Ensures unit compliance with all rules and regulations of the Agency, NYC Children’s Services, NYS Office of Children and Family Services, and all regulatory bodies related to foster care and adoption.
  • Works cooperatively with all other Agency departments to achieve common goals.
  • Supervises Case Planners to ensure they conduct ongoing face‑to‑face contacts with the child, parent, and foster parent on caseload, adhering to SCO casework contact policies.
  • Ensures timely completion of documentation in Connections within 7 business days of the event date.
  • Assesses safety and risk as well as protective and promotive factors and collaborates with families to create safety plans.
  • Partners with the child/family to continuously identify strengths, resources, and supports needed, engaging families in the development and assessment of their service plan.
  • Makes referrals for services for children and families, both internally and externally.
  • Works side‑by‑side with the foster parent and birthparent to support and provide treatment to the child in their care.
  • Collaborates with parent advocates to support parents in addressing personal and/or family challenges so children can safely return home.
  • Engages parents and foster families in concurrent planning from the beginning of the case, and works with foster parents toward adoption or KinGAP as appropriate based on each child’s permanency goal.
  • Attends all required trainings including Core Case Planning and TRIPP; attends additional trainings, seminars, and workshops as needed.
  • Completes timely comprehensive Permanency Hearing reports and FASP reports regarding the family's action plan and progress toward permanency.
  • Within 30 days of placement, completes initial treatment plan.
  • Intervenes in crisis situations to help the child and family stabilize.
  • Reinforces foster parent trainings.
  • Counts and monitors medication and medication compliance.
  • Attends and testifies in court appearances.
  • Supervises and oversees family visits between parents and children as needed.
  • Hold interdisciplinary treatment teams every 30 days for the first 90 days of placement to assess the child’s adjustment and progress; after 90 days, the treatment team meets at a minimum quarterly.
  • Reviews all FASPs, Progress Notes, Court Reports, correspondence and other documentation, ensuring quality and timely submissions.
  • Ensures timely and accurate submission of all forms and documents required by ACS.
  • Facilitates case planning meetings that include family, case planners, and clinical staff.
  • Provides coverage for Case Planners during PTO or when there are multiple vacancies.
  • Fulfills the duty of Mandated Reporter, reporting any suspected child abuse or maltreatment.
  • Meets with Case Planners one‑on‑one supervision at minimum bi‑weekly to discuss safety, permanency, and well‑being issues on all assigned cases.
  • Completes supervision notes in Connections within 7 business days of the event date.
  • Communicates and works in collaboration with programming staff.
  • Responds timely to all hotline and emergency calls, including nights and weekends.
  • Maintains complete, accurate and organized case records as well as program records such as time and attendance.
  • Attends supervisory core training and TRIPP; stays aware of significant changes in the field and attends additional training as needed.
  • Completes staff evaluations, resolves staff issues, participates in hiring and disciplinary actions.
  • Performs all other duties as assigned by the EFFC Director and Vice President.
Qualifications
  • Master’s degree required; MSW preferred and a minimum of two (2) years in child welfare.
  • Approved background clearance and fingerprinting required.
  • Excellent oral and written communication skills.
  • Proficiency in computer skills, including Microsoft Office, Excel and Connections.
  • Bilingual abilities strongly encouraged.
  • Valid NYS Driver License required.

  • Sets limits and maintains the helping role of practitioner, intervening appropriately.
