Senior Solutions in Vermont is seeking a compassionate Case Manager to support older adults and people with disabilities in the White River Junction/Windsor area. You will work with a person-centered planning approach, collaborating with community partners, and navigating state and federal benefits.

The role requires strong organizational and communication skills, a Bachelor's degree in human services or related experience, and a valid driver’s license.

Responsibilities

• Collaborating with other community partners to create a person-centered plan of care.

• Assisting, when necessary, to gain access to existing services such as public benefits, housing legal services, etc.
• Ensuring that all provided services does not exceed budgeted amounts and are consistent with DAIL guidelines.

At Senior Solutions (Area Agency on Aging in Southeast Virginia), we are seeking a Case Manager to work with older adults and/or people with disabilities in the White River Junction/Windsor and the surrounding areas using a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are required. Qualifications: Bachelor's degree in human services and prior case management experience preferred.
