JWCH Institute, Inc. is seeking a Case Manager in Los Angeles to deliver comprehensive case management for patients from JWCH, Wesley Health Centers, and Kaiser ED referrals. The role focuses on needs assessment, care planning, service coordination, and patient advocacy to improve outcomes.

The position requires strong organization, communication, and care coordination skills; EHR experience with Next-Gen is preferred; bilingual English/Spanish is a plus.

Qualifications

• Bachelor's degree in social work or related field; 1+ year care coordination experience preferred.
• High school diploma with 3–4 years in a care coordination/housing navigator role preferred.
• Bilingual English/Spanish preferred.
• Experience with EHR systems, Next-Gen.
• Experience in DMH funded programs preferred.

Responsibilities

• Provide comprehensive case management to JWCH, Wesley Health Centers and referred patients.
• Develop and monitor care plans, coordinate services and advocate for patients.
• Coordinate referrals across medical, behavioral health, housing and transportation services.
• Maintain accurate patient records and timely documentation; prepare reports.

Salary Range: $48,579.00 To $53,622.00 Annually

Location: East Hollywood, CA. Vermont Clinic

Position Purpose:The Case Manager will provide comprehensive case management services to patients of JWCH, Wesley Health Centers and those referred from the Kaiser Emergency Department (ED) program. This role involves assessing patient needs, developing care plans, coordinating services, and advocating for patients to ensure they receive the necessary care and support. The Case Manager will work collaboratively with healthcare providers, social services, and community resources to improve patient outcomes and enhance the quality of care.

• Conducts comprehensive case management services, including intake and assessment, development of a comprehensive care plan, implementation of the care plan, specialty referrals, and follow-up.
• Monitor and adjust care plans as needed to ensure optimal patient outcomes.
• Connects clients to support services and provides appropriate referrals including but not limited to medical, addiction medicine and recovery programs, dental, transportation and housing.
• Ensure timely follow-up on referrals and appointments.
• Collaborate with multidisciplinary team to address the needs of persons living with mental illness, substance abuse disorders, complex medical conditions, or experiencing or at risk of homelessness
• Advocates on behalf of clients with healthcare professionals, utilizing motivational interviewing, trauma-informed care, and harm-reduction approaches
• Coordinate a range of medical and psychosocial services to address social, emotional, and practical needs
• Assist patients in navigating healthcare services and overcoming barriers to care.
• Educate patients and families on available resources and support services.
• Maintain accurate and up-to-date patient records, including assessments, care plans, and progress notes.
• Prepare and submit required reports and documentation in a timely manner
• Coordinate outreach activities in and outside of the clinic
• Maintain productivity requirements to meet contract compliance.
• Complete and maintain daily documentation to DMH contract standards in a timely manner.
• Participate in team meetings, case conferences, and other collaborative activities.
• Communicate effectively with patients, families, and healthcare team members.
• Participate in the development and implementation of case management programs and initiatives.
• Perform other duties as assigned.

Requirements:

• AA or Bachelor's degree in Social Work, Sociology, or a related field, with at least 1 year of experience as a care coordinator, community health worker, or in a similar role preferred
• High School Diploma or GED, with at least 3-4 years of experience as a care coordinator, housing navigator, community health worker, or in a similar role.
• Willing and able to work in low-income areas of Los Angeles with persons experiencing mental illness and are homeless or at risk of homelessness.
• Strong assessment and care planning skills.
• Excellent organizational and time-management skills.
• Effective communication and interpersonal skills.
• Experience with Electronic Health Recording (EHR), Next-Gen.
• Bachelor's degree in social work or equivalent experience is preferred.
• Bilingual (English/Spanish) ability is preferred.
• Experience working in a DMH funded program is preferred.
