Job summary

Valley Behavioral Health is seeking a Case Manager to support clients in building practical skills and moving toward meaningful goals. You will provide psychosocial rehabilitative services, help with care plans, and ensure documentation and regulatory standards are met.

The role emphasizes client-centered support, collaboration with a multidisciplinary team, and compliance with Medicaid guidelines while offering growth opportunities and a stable on-site schedule.

Qualifications

• High School diploma or equivalent.
• Case Manager certification within 120 days of start date.
• CPR certification (post-hire).
• Valley de-escalation certification (post-hire).
• Driving positions require a valid driver’s license and age 21+.

Responsibilities

• Provides psychosocial rehabilitative services as described by the Medicaid manual.
• Supports clients in following individualized care plans and identifying case management goals.
• Completes documentation on services provided within Valley’s standards.
• Meets productivity expectations as assigned by supervisor.
• Distributes medications as necessary.
• Transports clients to appointments as necessary.
