Job summary

Community Bridges Inc. in Springerville, AZ seeks a Case Manager to coordinate treatment services for children, adolescents, and adults within our Behavioral Health/Integrated Care program.

You will provide ongoing support, ensure services prior to discharge, and help remove barriers to community reintegration, while engaging families and collaborating with the treatment team. This role offers comprehensive benefits and strong opportunities for professional growth.

Qualifications

  • High School Diploma or GED required; Associate’s Degree preferred.
  • 1-3 years of health care related work experience in behavioral/medical crisis is preferred but not required.
  • 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Minimum of six months of recovery from substance use or mental health disorders preferred.
  • Experience working with children/adolescents preferred.
  • Arizona Fingerprint Clearance card required; AZ Driver’s License in good standing; clear MVR.

Responsibilities

  • Provide ongoing support and coordination with treatment teams to address child/adolescent needs.
  • Identify needs and ensure supportive services are provided prior to discharge from CBI Behavioral Health/Integrated Care.
  • Engage patients and families to reduce barriers to treatment and improve community outcomes.
  • Work with adults in similar capacity to address behavioral health needs and goals.

Case Manager
Job Summary

The Child/Adolescent Integrated Case Manager is an individual who assists inaddressing all the ancillary concerns and needs of the child/adolescent by providingongoing support, needs identification, and intra-agency/external coordination inconjunction with the established treatment team. This employee is responsible forensuring supportive and follow-up services are provided prior to discharging apatient/participant from a CBI Behavioral Health/Integrated Care program. The goal ofthe Child/Adolescent Integrated Case Manager is to enhance the patient’s likelihood ofsuccess in the community and to identify and reduce barriers to treatment. TheChild/Adolescent Integrated Case Manager will provide support and ongoing patientengagement while the adolescent is participating in CBI Services. The employee in this position will also work with adults, providing similar services as summarized above, to address client specific behavioral health needs, objectives, and goals.

Skills/Requirements
  • High School Diploma or GED is required. Associate’s Degree is preferred.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
  • 1-3 years of substance abuse/mental health recovery-oriented service is preferred but not required.
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred.
  • Experience working with children/ adolescents preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures, is required.
  • Arizona Fingerprint Clearance card required.
  • Current AZ Driver’s License (valid and in good standing)
  • Clear 39-month Motor Vehicle Record (MVR)
