Job summary

WellMed at Eastfield in Texas is seeking a Registered Nurse Case Manager to coordinate care for high-risk patients, perform assessments, reconcile medications, and support motivational interviewing and self-management goals. BLS certification and an active RN license are required, with 2+ years in clinical settings.

You will transition patients from inpatient to outpatient care, update plans in the EMR, and participate in PCC meetings while maintaining evidence-based practice.

Qualifications

  • Bachelor’s degree in Nursing or Associate’s degree in Nursing with 4+ years RN experience.
  • Active and unrestricted RN license in state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician’s office, clinical or hospital setting.
  • Knowledge of COPD/asthma, diabetes, CHF and IHD.
  • Proficient with electronic medical records.
  • Excellent verbal and written communication.
  • Strong interpersonal and teamwork skills.
  • Ability to interact productively with multidisciplinary teams.
  • Strong organizational and prioritization skills.
  • TB screening and immunity proof per policy.

Responsibilities

  • Identify high-risk patients with providers and clinic staff.
  • Support longitudinal care with assessment, medication reconciliation, motivational interviewing, and self-management goals.
  • Coordinate transitions from inpatient to outpatient and ensure post-discharge needs.
  • Update plan of care with providers and maintain EMR documentation.
  • Participate in PCC meetings and weekly reports.
  • Maintain current nursing practice and evidence-based knowledge.
  • Perform additional duties within scope as needed.

Primary Responsibilities:
  • Works with the providers and clinic staff to identify patients at high risk.
    • Supports longitudinal care of the patient with chronic care conditions by:
    • performing assessment of health conditions
    • performing medication reconciliation
    • conducting Motivational Interviewing and Self-Management Goal setting
    • providing patient educationcreating referrals to appropriate agencies and resources
  • Supports transition of the patient with chronic care conditions from inpatient to outpatient setting, by:
    • performing assessment of transitional needs
    • performing medication reconciliationestablishing and reviewing contingency plan
    • providing patient education
    • assisting with post discharge needs such as prescriptions, transportation, Durable Medical Equipment (DME), appointments
  • Coordinate with providers to establish or update plan of care
  • Performs accurate and timely documentation in the electronic medical record
  • Participates in daily huddles and Patient Care Coordination (PCC) meetings
  • Prepares accurate and timely reports, as required, for weekly meetings
  • Maintains continued competence in nursing practice and knowledge of current evidence based practices
  • May perform clinical tasks within their scope of practice
  • Performs all other related duties as assigned

Required Qualifications:
  • Bachelor’s degree in Nursing or Associate’s degree in Nursing with 4+ years of experience as an RN
  • Registered Nurse with an active and unrestricted license to practice in the state of employment
  • Current BLS certification
  • 2+ years of experience in a physician’s office, clinical or hospital setting
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
  • Proficient computer skills to work efficiently with electronic medical records
  • Demonstrated excellent verbal and written skills
  • Demonstrated solid interpersonal skills
  • Demonstrated ability to interact productively with individuals and with multidisciplinary teams
  • Demonstrated excellent organizational and prioritization skills
  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
Preferred Qualifications:
  • Experience related to patient education and/or motivational interviewing skills and self-management goal setting
  • Fluent written and verbal skills in English and Spanish
Physical & Mental Requirements:
  • Ability to lift up to 35 pounds
  • Ability to push or pull heavy objects using up to 35 pounds of force
  • Ability to sit for extended periods of time
  • Ability to stand for extended periods of time
  • Ability to use fine motor skills to operate office equipment and/or machinery
  • Ability to receive and comprehend instructions verbally and/or in writing
  • Ability to use logical reasoning for simple and complex problem solving

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $60,200 - $107,400 annually based on full-time employment. We comply with all minimum wage laws as applicable.
