Select Specialty Hospital is seeking a Case Manager for a full-time, Mon-Fri 8-5 role at our LTACH. Requires licensure in a clinical discipline (RN/LPN/LVN or RT) or Social Work per state guidelines; inpatient discharge planning experience preferred.
You will lead utilization reviews, discharge planning, treatment plan and financial management, document medical records, and collaborate with the care team to ensure timely, patient-centered outcomes.

Requires a current licensure in a clinical discipline either as a Nurse (RN/LPN/LVN) or a Respiratory Therapist (RT) OR Medical Social Work (license per state guidelines).
And
Previous discharge planning experience preferred.

You will report directly to the Director of Case Management and provide social work services, as necessary, per state guidelines.
