Job summary

UnitedHealth Group is seeking a Case Manager RN in Boulder, CO to coordinate patient care across the continuum. This onsite, full-time role emphasizes interdisciplinary collaboration, discharge planning, and regulatory compliance to improve patient outcomes.

You will lead post-hospital assessments, educate patients and families, ensure smooth transfers, and work closely with physicians and case managers to develop optimal care plans in a dynamic hospital setting.

Qualifications

• RN license in Colorado, OR LCSW in Colorado, OR MSW with ability to obtain within 6 months.
• 2+ years of experience as a clinician in an acute care setting with hospital workflow knowledge.
• Ability to obtain required licenses within 6 months of hire.

Responsibilities

• Coordinate post-hospital assessments and determine discharge plans for complex cases.
• Educate patients and families on care options and complete necessary paperwork.
• Facilitate orders to prevent discharge delays when transferring to the next level of care.
• Collaborate with physicians, families, and case managers to determine discharge plans.
• Report information to staff and comply with CMS and Joint Commission standards.
• Remain flexible to support hospital needs, including floating to other departments as needed.

About This Role

The Case Manager RN is responsible for coordinating patient services through an interdisciplinary process, focusing on clinical and psychosocial approaches throughout the continuum of care. What You'll Do

• Integrate assessment of post-hospital services and determine appropriate discharge plans for complex cases.
• Educate patients/families on options within the determined level of care and ensure completion of necessary paperwork.
• Facilitate completion of orders prior to patient transfer to the next level of care to avoid discharge delays.
• Emphasize education and collaboration with physicians, family members, and case managers to determine beneficial discharge plans.
• Report relevant information to staff assuming responsibility in the next level of care.
• Comply with all regulatory requirements, including CMS and Joint Commission Standards.
• Demonstrate knowledge of federal and state regulations and managed care.
• Consult with physician section leaders for support in cases of unresolved continued stay.
• Communicate with department director regarding length of stay and financial information.
• Remain flexible to meet hospital needs, including floating to other departments as necessary.

Requirements

Education:

• Registered Nurse (RN) with a current, active, unrestricted RN License in Colorado, OR a Licensed Clinical Social Worker (LCSW) in Colorado, OR a Master of Social Work (MSW) with ability to obtain within 6 months of hire.

Experience:

• 2+ years of experience working as a clinician in an acute care setting with proficient knowledge of hospital workflows.

Licenses & Certifications:

• Current RN License in Colorado, OR
• LCSW (Licensed Clinical Social Worker) - required, OR
• MSW (Master of Social Work) - required, OR
• Ability to obtain required licenses within 6 months of hire.
