Job summary

The Eureka, California facility seeks a Case Manager Registered Nurse to support patient care coordination, discharge planning, and utilization review on day shift. This onsite role is a 26-week contract starting 11/02/2026.

You'll work 8-hour shifts, 5 days a week, with travel experience required and Epic charting. Local candidates only; RTO restrictions apply. Must have at least 2 years of acute hospital case management experience.

Qualifications

• 2 years of acute hospital case management experience.
• Experience with care coordination and discharge planning.
• Travel willingness required; first-timers accepted with approval.
• Epic charting knowledge is required.
• Experience with CMS guidelines and DRGs.

Responsibilities

• Coordinate patient care from admission to discharge.
• Perform utilization review and authorize care.
• Develop and implement discharge plans.
• Coordinate with physicians, families, and social services.
• Complete documentation in Epic.
