Albert Einstein Medical Center is seeking a Case Manager RN to coordinate comprehensive care for a defined patient population and to facilitate discharge planning and safe discharge for assigned patients.

The role emphasizes advocacy, communication across the care team, and proactive progression of patient care from admission to discharge, with collaboration across disciplines and involvement in daily care progression activities.

Qualifications

  • Bachelor’s Degree in Nursing with 3 years of case management or discharge planning experience.
  • Demonstrates effective conflict resolution skills as necessary.
  • Uses interdisciplinary team approach for patient care planning and discharge coordination.
  • Understands impact of developmental stage on illness/trauma adjustment.
  • Identifies adolescent behaviors related to body image, independence, and identity.
  • Understands end-of-life issues and palliative considerations.
  • Communicates effectively with neonate, infant, adolescent, and geriatric patients.
  • Recognizes signs of abuse or neglect to help identify victims.
  • Identifies hearing/sight deficits in geriatric patients.

Responsibilities

  • Coordinate discharge planning and establish safe discharge plans.
  • Advocate for the patient and family throughout the episode of care.
  • Act as a resource to facilitate communication among the healthcare team and patient/family.
  • Monitor patient progress and facilitate movement through the hospital stay.
  • Review length-of-stay and barriers data to support rounds and escalation meetings.
  • Lead or support the Patient Progression Leader in daily huddles and care progression.
  • Escalate unresolved barriers to Department Leadership.

Job Description Summary The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population to facilitate patients through the continuum of care. Coordinates all aspects of discharge planning and establishes a safe discharge plan for assigned patients.

Job Duties
  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
  • Advocates for the patient and family throughout the entire episode of care.
  • Functions as a resource to facilitate communication among the healthcare team and patient/family.
  • Facilitates patient progression throughout the patient’s hospital stay.
  • Reviews patient length of stay/barriers data to facilitate patients’ progression during Multidisciplinary Rounds and Escalation/Outlier meetings.
  • Reviews patients’ status daily to identify issues requiring priority interventions or attention which may impact the established discharge plan.
  • May facilitate daily patient Plan of Care Huddle Rounds functioning as the Patient Progression Leader or supports the identified Patient Progression Leader.
  • Monitors patient’s progress intervening as necessary to ensure patient progresses efficiently.
  • Proactively identifies and resolves barriers to patient care and/or progression to discharge.
  • Escalates unresolved barriers to Department Leadership.
Minimum Qualifications
  • Bachelor’s Degree of Nursing 3 years case management or discharge planning experience

  • Understands the impact of developmental stage on adjustment to illness/trauma.
  • Demonstrates ability to identify adolescent behaviors concerning body image, independence, sexual identity, interference with ability to establish identity, tendency to deny or minimize severity of illness.
  • Demonstrates understanding of end of life issues.
  • Exhibits effective communication skills while working with neonate, infant, adolescent, and geriatric patient.
  • Demonstrates ability to recognize the signs of physical and/or emotional abuse or neglect to help identify victims.
  • Demonstrates the ability to identify hearing and sight deficiencies in geriatric population.

Additional Qualifications

Physical Demands Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
