Job summary

Singing River Health System in Pascagoula, MS, is seeking a Case Manager RN for Medicaid. You will coordinate patient care, focusing on clinical quality, resource management, and streamlined discharge planning, partnering with inpatient and outpatient teams.

You will evaluate Medicaid client needs, develop personalized care plans, and ensure alignment with SRHS policies and guidelines. A BSN is preferred; Mississippi RN license is required; BCLS and de-escalation training are also expected

Qualifications

• Graduate of an accredited School of Nursing; Bachelor’s Degree in Nursing preferred.
• Current Mississippi RN license.
• BCLS certification upon hire and/or transfer; TB certification preferred; de-escalation training by orientation end date (90 days).
• Two years’ nursing experience in ambulatory or acute care relevant to Case Management.

Responsibilities

• Coordinate patient care to improve clinical quality and manage resources.
• Participate in daily discharge planning meetings; identify barriers to discharge.
• Ensure communication between inpatient care management and outpatient case management teams.
• Maintain professional standards and SRHS policies in all duties.

They coordinate patient care to a population of different ages within the Singing River Health System. Expectation is for all performed duties to be in accordance with Singing River Health System procedures and policies, accreditation organization, and governing guidance and publications for health care employees. If responsible for Medicaid patients, evaluates Medicaid client needs, builds personalized care plans, and coordinates home and community-based services.Case Manager must be able to work effectively under pressure, multitask and be self-directed. Case Managers must have strong written and oral communication skills, analytical and critical thinking skills, conflict management and team-building skills, and strong organizational skills. The Case Manager must have the ability to perform numerous tasks simultaneously with minimal errors. Case Managers will participate in the daily discharge planning meetings: addressing barriers that may inhibit discharge; targeting successful transition from one level of care to the next. TB certification preferred.Must have de-escalation training completed by the end of position orientation (90 days); must have appropriate level of de-escalation training. **Experience:** Two (2) years’ nursing experience in ambulatory or acute care that has relevance to the practice of Case Management.**Reports to:** Director- Patient Resource Management**Supervises:** None.**Physical Demands:** Work is moderately active: involves sitting with frequent requirements to move about the office, move about the facility, and to travel to another facility within the SRHS service area. Work involves exerting a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. Work involves using many physical motions in performing daily work activities; subject to exposure of body fluids, sputum and tissues, which may carry the hazard of infectious disease. Work involves using repetitive motions: substantial movements of the wrists, hands, and or fingers while operating standard office equipment such as computer keyboard. Work involves being able to perceive the nature of sound at normal speaking levels with or without correction; the ability to make fine discriminations in sound. Work requires close visual and acuity and the ability to adjust the eye to bring an object into sharp focus, i.e. shift gaze from viewing a computer monitor to forms/printed material that are closer to compare data at close vision. Must be able to be active for extended periods of time without experiencing undue fatigue. Must be able to work schedules assigned with the understanding that changes may be instituted according to the needs of the hospital for off days, shifts or weekends.**Mental Demands:** Must demonstrate keen mental faculties/assessment and decision making abilities. Must demonstrate superior communication/speaking/enunciation skills to receive and give information in person and by telephone. Must demonstrate strong written and verbal communication skills. Must possess emotional stability conducive to dealing with high stress levels.
