Job summary

Somatus is seeking a Field-Based Nurse Case Manager (RN) to support high-risk patients in homes, dialysis clinics and physician offices. You will lead care planning, conduct assessments, review medications and engage nephrology practices to improve outcomes.

This 75% travel role covers the greater Seattle area, combining in‑person visits with strong communication to the care team. A WA RN license, 3+ years of renal care experience, and MS Office proficiency are required.

Qualifications

• Current, unrestricted RN license in the state of Washington or ability to obtain
• 3+ years of RN experience, with renal/kidney care focus
• Proficiency with MS Office; strong data entry and Windows navigation
• Willingness to travel up to 75% to homes, clinics and facilities
• Ability to establish trust with patients, caregivers and providers
• Experience working in managed care environments is a plus

Responsibilities

• Outreach and schedule member visits in homes, dialysis clinics or offices
• Conduct in‑home and in‑facility visits to support care plans
• Document findings and coordinate with interdisciplinary teams
• Assist with medication reviews and care planning
• Identify barriers to care and engage resources to improve outcomes
• Complete field safety trainings and maintain required certifications

How You’ll Make an Impact

Field‑Based Nurse Case Managers are an important part of the interdisciplinary care team. They are responsible for outreaching and scheduling member face‑to‑face visits in homes, dialysis clinics, or physician offices to support higher‑risk members needing barrier assessments and face‑to‑face care plan education. In addition, field nurses develop trusting relationships with nephrology practices in their markets and work closely with those practices to support improvement in members’ health outcomes.

This is a field‑based position meeting with members in their homes and facilities and will be in the field at least 75% of the time. The territory will primarily cover the greater Seattle area.

• Provide a complete continuum of quality care through close communication with members via in‑person or on‑phone interaction
• Travel to complete in‑home and in‑facility visits with members
• Travel to participate in care planning in facilities & provider offices
• Use nursing assessment skills to identify medical, behavioral, social determinants of health gaps or barriers in treatment plan
• Provide condition education, conduct medication reviews, assist with advance planning and connect to community resources
• Demonstrate ability to establish trusting relationships with members & caregivers and identify innovative strategies to improve and maintain engagement
• Participate in interactions with interdisciplinary team, treating providers and community resources
• Complete in‑patient & post‑discharge visits to ensure that discharged members receive the necessary services and resources to focus on reducing readmissions
• Document and track findings; solid computer and software navigation skills are critical
• Complete and maintain field‑based safety trainings

Qualifications

• Current, unrestricted RN license in the state of Washington
• Ability to get licensed in other states as needed
• 3+ years of RN experience
• Renal, Chronic Kidney Disease or Dialysis care experience as a main focus of your job
• Computer proficiency, including strong data entry, utilizing MS Office (Word, Excel, PowerPoint and Outlook), and ability to type and talk at the same time while navigating a Windows environment
• Reside in a location that can receive a high‑speed internet connection or can leverage existing high‑speed internet service

Preferred Qualifications

• BSN or higher level of education
• Certified Case Manager (CCM)
• Diabetic Educator experience
• Field‑based experience going into homes
• Telephonic case management experience
• Experience working within a managed care environment

Compensation

$85,000 to $97,000 per year.
