Somatus is seeking a Field-Based Nurse Case Manager (RN) to support high-risk patients in homes, dialysis clinics and physician offices. You will lead care planning, conduct assessments, review medications and engage nephrology practices to improve outcomes.
This 75% travel role covers the greater Seattle area, combining in‑person visits with strong communication to the care team. A WA RN license, 3+ years of renal care experience, and MS Office proficiency are required.

Field‑Based Nurse Case Managers are an important part of the interdisciplinary care team. They are responsible for outreaching and scheduling member face‑to‑face visits in homes, dialysis clinics, or physician offices to support higher‑risk members needing barrier assessments and face‑to‑face care plan education. In addition, field nurses develop trusting relationships with nephrology practices in their markets and work closely with those practices to support improvement in members’ health outcomes.
This is a field‑based position meeting with members in their homes and facilities and will be in the field at least 75% of the time. The territory will primarily cover the greater Seattle area.
$85,000 to $97,000 per year.
