The Case Manager applies each of these six essential activities within each of the following five core components: Coordination and Service Delivery, Physical and Psychological Factors, Benefit Systems and Cost Benefit Analysis, Case Management Concepts, and Community Resources. These activities and core components are applied to promote wellness on the continuum of care.
