ContinueCARE Hospital in Kentucky seeks a nurse case manager to direct and coordinate the integration of Case Management into the patient care, discharge, and home planning processes across departments.

The role emphasizes collaboration with families and care teams to maximize benefits from health care services and to support patients during a 25-day average stay in specialty programs within ContinueCARE's small-hospital environment embedded in a large acute care system.

Qualifications

• Graduate of an accredited nursing program.
• BSN preferred.
• Unrestricted RN license.
• Case Management experience in an acute care environment preferred but not required.

Responsabilités

• Direct and coordinate the integration of Case Management into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies and health care facilities.
• Coordinate the provision of clinical care to patients, families and significant others to help them cope with the impact of illness and to maximize benefits from health care services.
• Support the ContinueCARE Hospital environment and participate in the care planning for patients with an average length of stay of 25 days in specialty programs.

Formation

RN license (Unrestricted)

BSN preferred

Directs and coordinates the integration of Case Management functions into the patient care, discharge, and home planning process with other hospital departments, external service organizations, agencies, and health care facilities. Coordinates the provision of clinical care to patients, families and significant others to enable them to deal with the impact of illness on individual family functioning and to achieve maximum benefits from health care services. At ContinueCARE Hospital, we take pride in the care we provide for patients with an average length of stay of 25 days in a variety of specialty programs. Our staff enjoy our small-hospital environment inside of a large acute care hospital.

Qualifications:
