Job summary

HCA Florida Brandon Hospital in Brandon, FL is seeking a Case Manager RN to guide patients through their care journey, coordinate services, and develop individualized care plans. You will partner with physicians, nurses, and the care team to ensure safe transitions and clear communication across the continuum of care.

The role requires an Associate Degree in Nursing or Nursing Diploma, RN license in Florida, and 2+ years in case management or 3+ years in clinical nursing.

Qualifications

• Associate Degree in Nursing or Nursing Diploma required.
• Bachelor’s degree in nursing preferred.
• 2+ year experience in case management OR 3+ years’ experience in clinical nursing required.
• Currently licensed as a Registered Nurse in state of practice.
• InterQual experience preferred
• Certification in Case Management preferred
• Ability to establish and maintain collaborative working relationships
• Ability to communicate effectively in oral, written and electronic formats
• Analytical and critical thinking abilities with proactive decision-making and negotiation skills

Responsibilities

• Guiding patients and families through program orientation, explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinating education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Developing and implementing individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
• Completing psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborating closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care
• Promoting HCA Healthcare’s values of compassion, respect, and excellence through every patient and family interaction

Job Summary and Qualifications
