Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field.
• Minimum 2 years of experience in human services, case management, social services, behavioral health, residential services, or related field.

Responsibilities

• Manage an assigned caseload and meet with residents regularly to understand their needs and goals.
• Build supportive, professional relationships with residents and advocate on their behalf.
• Connect residents with healthcare, behavioral health, housing, social services, benefits, and community resources.
• Coordinate and accompany residents to medical, psychiatric, and other appointments as needed.
• Help residents navigate healthcare and social service systems.
• Develop and maintain individualized service plans.
• Monitor resident progress and identify barriers to goals.
• Lead at least two ongoing resident groups or activities.
• Provide support during crisis situations and connect residents with emergency resources when needed.
• Maintain confidential records and documentation.
