Job summary

Atrium Health is seeking a Registered Nurse with a BSN and in some cases a Master’s preference for advanced care coordination and patient management. The role emphasizes case management and discharge planning across various care settings and requires licensure in the state of practice.

Two years of healthcare experience and certification within 3 years are part of the expectations, along with strong communication and organizational skills and proficiency with applicable tools. BLS is required.

Qualifications

• BSN required; Masters preferred.
• Current RN license or temporary license as an RN Petitioner in the state of work and residence, or licensure in home state if National License Compact state is used.
• If declaring a National License Compact state as residency, licensure must be in home state.
• For Non-NLC states, current RN license or temporary license as RN Petitioner in the state where the RN works.
• Two years' experience required in health care.
• Experience includes case management/discharge planning in acute care, home care, LTC, physician office, or managed care company.
• Certification required within 3 years of hire per departmental protocol.
• Clinical competence in disease management and case management principles.
• Excellent interpersonal communication, negotiation skills, problem-solving skills, organizational and time management skills; ability to work independently and with the care team.
• Knowledge and proficiency in appropriate tools.
• BLS required per policy guidelines.

Two years' experience required in health care. Experience includes case management/discharge planning in one of the following settings: Acute care, Home care, LTC care, Physician Office, or Managed Care company. Appropriate professional certification required within 3 years of hire date for professional certification per departmental protocol. Clinical competence in disease management and case management principles. Must possess excellent interpersonal communication and negotiation skills, problem-solving skills, strong organizational and time management skills, and the ability to work independently and as a member of the care team. Requires demonstrated knowledge and proficiency in appropriate tools. BLS required per policy guidelines.
