Job summary

CVS Health is seeking a Case Manager, Registered Nurse, to deliver community-based care management. The role emphasizes compassionate communication and coordinating care for members in a remote GA setting, with a focus on outcomes and patient education.

The position requires an RN with at least an Associate's degree, with a Bachelor’s preferred, and 3+ years of clinical practice plus 2+ years case management. Eligible candidates must hold a valid compact-state RN license.

Qualifications

• Associate's Degree in Nursing (RN) required.
• Bachelor's Degree in Nursing preferred.
• 3+ years clinical practice, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, home health with Medicare.
• 2+ years case management or discharge planning experience.
• Active and unrestricted RN license in a compact state.

Responsibilities

• Facilitate the delivery of appropriate benefits and healthcare information.
• Develop, implement, and support health strategies and policies.
• Act as a liaison with members, employers, providers, and insurance companies.
• Implement and coordinate case management activities for catastrophic cases and chronically ill members.
• Assess and analyze medical and vocational status of members.
• Communicate with members and stakeholders regarding care and treatment.
• Prepare documentation of case work activities.
• Consult with internal multidisciplinary teams for optimal health outcomes.
• Provide educational and prevention information for best medical outcomes.
• Monitor member progress toward desired outcomes.

Experience:

• 3+ years clinical practical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, home health with Medicare members.
• 2+ years case management, discharge planning, and/or home health care coordination experience.

Licenses & Certifications:

• Active and unrestricted Compact Registered Nurse (RN) license and live in compact state.
