Working closely with physicians, nurses, social workers, and other interdisciplinary partners, you'll help identify barriers to discharge, connect patients with appropriate resources, and promote continuity of care across acute, post-acute, and community settings. This is an opportunity to make a meaningful impact on the patient experience while supporting quality, operational efficiency, and responsible healthcare resource utilization.

As an RN Case Manager, a typical day may include:

  • Conducting comprehensive patient assessments and documenting clinical findings, discharge needs, and care coordination activities within the electronic health record

  • Participating in interdisciplinary rounds to present patient cases, communicate discharge planning updates, and collaborate on appropriate next steps

  • Identifying potential barriers to discharge and proactively coordinating solutions to support patient progression, appropriate length of stay, and efficient transitions of care

  • Assessing healthcare-related social needs and social determinants of health to connect patients and families with appropriate community resources and support services

  • Coordinating timely referrals for post-acute services, including durable medical equipment, home care, rehabilitation, and facility placement

  • Maintaining accurate, timely, and compliant documentation while ensuring adherence to CMS Conditions of Participation, HIPAA, patient rights, and organizational policies

  • Participating in quality improvement initiatives focused on reducing length of stay, improving patient throughput, strengthening departmental performance, and enhancing the patient experience

This role might be for you if:

  • You are an experienced Registered Nurse who enjoys combining clinical expertise with patient advocacy, care coordination, and problem-solving

  • You understand the importance of safe discharge planning and helping patients successfully transition between healthcare settings

  • Strong nursing assessment and critical thinking skills allow you to identify complex patient needs and develop practical, individualized solutions

  • You enjoy collaborating with physicians, nurses, social workers, and other interdisciplinary professionals to coordinate patient care

  • You are comfortable navigating utilization review, medical necessity criteria, insurance authorizations, and healthcare reimbursement processes

  • You can identify barriers to patient progression and work proactively to support timely discharge and appropriate resource utilization

  • You communicate clearly and compassionately with patients and families, including during challenging conversations involving discharge planning or complex care needs

  • You understand how social determinants of health influence patient outcomes and are committed to connecting individuals with appropriate support services

  • You are organized, adaptable, and comfortable managing multiple patients, competing priorities, and time-sensitive responsibilities

  • You value accurate documentation, regulatory compliance, and evidence-based clinical decision-making

  • You enjoy using performance data and quality improvement initiatives to strengthen patient outcomes and healthcare operations

  • You bring cultural sensitivity, professionalism, and a collaborative approach to supporting patients and families from diverse backgrounds

To be considered for this RN Case Manager opportunity, you must hold a Bachelor of Science in Nursing (BSN) and possess a current New Jersey Registered Nurse (RN) license.
