Job summary

Mission Hospital in Asheville, NC is seeking a Case Manager RN for PRN opportunities to coordinate inpatient and observation care and drive effective discharge planning.

You will collaborate with Social Services and physicians to ensure appropriate post-acute care, assess psychosocial and medical needs, document interventions, and monitor care transitions throughout the episode.

Qualifications

• 2+ years experience in Case Management OR 3+ years experience in a clinical setting.
• InterQual experience preferred.
• Associates Degree in Nursing is required.
• Masters Degree in Nursing preferred.
• Licensed as a Registered Nurse in the state of practice.
• BLS certification required.

Responsibilities

• Provide case management for inpatient and observation patients.
• Identify patients at risk during transitions between levels of care.
• Perform psychosocial, medical and discharge needs assessment for patients and families.
• Coordinate plan of care and discharge planning with the care team and physician.
• Partner with Social Services to ensure appropriate post-acute care.
• Evaluate care progression using InterQual criteria and escalate as needed.
• Serve as liaison between physicians, patients, families, hospital staff and outside agencies.
• Document case management activities and actions to meet care goals.
• Organize and document resources needed to accomplish the plan of care and discharge.
• Monitor information gathering to evaluate plan effectiveness and outcomes.
• Assess and document care management plan effectiveness at intervals.

Job Summary and Qualifications

· You will provide case management services for both inpatient and observation patients as assigned.

· You will Identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another

· You will perform a comprehensive assessment of psychosocial, medical and discharge needs of patients/family along with an assessment of resources appropriate and available to the patient/family

· You will coordinate the plan of care and drives the discharge plan by collaborating with the multidisciplinary health care team and in particular with the patient's physician to facilitate a successful care transition

· You will partner with Social Services to ensure the post-acute medical needs and level of care are appropriate

· You will evaluate progression of care using evidence-based tools and approved criteria (InterQual) throughout the episode of care; escalates progression and transition of care issues through the established chain of command

· You will act as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff, and outside agencies

· You will assess the process of collecting in-depth clinical information about a patient and identify individual needs in order to place the patient in this facility.

· You will plan the process of collaborating with interdisciplinary healthcare providers to determine specific objectives, goals and actions designed to meet the patient’s needs.

· You will implement the process of documenting specific case management activities/interventions that will lead to accomplishing the goals set forth in the patient’s care of plan.

· You will coordinate the process of organizing, securing, integrating, modifying and documenting the resources necessary to accomplish the goals of the plan of care and discharge.

· You will monitor the ongoing process of gathering sufficient information from all relevant sources and it’s documentation regarding the case management plan and it’s activities/ services to enable the case manager to determine the plan’s effectiveness.

· You will evaluate the process, repeated at appropriate intervals, of determining and documenting the care management plan effectiveness in reaching desired outcomes and goals.

· You will measure the intervention to determine the outcomes of care management involvement.

What qualifications you will need:

• Required Experience:2+ years experience in Case Management OR 3+ years experience in a clinical setting
• Preferred Experience:InterQual experience
• Required Education:Associates Degree in Nursing
• Preferred Education:Masters Degree in Nursing
• Required License:Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
• Required Certification:BLS
