Position Summary

The Case Manager Registered nurse is a work from home position with travel required (in Maricopa County, Arizona) to visit members. This person utilizes skills to coordinate, document, and communicate all aspects of the utilization/benefit management program. They apply critical thinking and knowledge in clinically appropriate treatment, evidence-based care, and medical necessity criteria for members by providing care coordination, support, and education through the use of care management tools and resources.

Required Qualifications

2+ years of experience in a clinical setting. Active RN license, valid in the state of Arizona. Must reside in Maricopa County, Arizona. Ability to travel up to 25% of the time within Maricopa County, Arizona.

Preferred Qualifications

2+ years of experience in case management. Strong organizational and time management skills. Ability to collaborate with both internal and external partners. Demonstrated proficiency in Microsoft Office Suite, including Outlook, Word, etc. Previous experience collaborating with medical professionals. Bilingual (English/Spanish). Bachelor’s degree in Nursing. Education Associate’s degree.

Pay Range

The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
