Job summary

UnitedHealth Group is seeking a per diem RN Case Manager for Hospice services, delivering compassionate, patient-centered care in home settings across Tennessee. You’ll assess, plan, and coordinate interdisciplinary care, guiding patients and families through the health journey with empathy and professionalism.

Qualifications

• Must hold an active RN license for the state(s) served.
• Current CPR certification.
• Minimum 1 year of nursing experience.
• Valid driver's license with reliable transportation; liability insurance may be required.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned

As Per Diem RN Case Manager, you’ll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs.

Primary Responsibilities:

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies
• Records pain/symptom management changes/outcomes as appropriate
• Teaches patients, families, and caregivers appropriate patient care techniques
• Regularly communicates with clinical manager and care team
• Performs on-call responsibilities and on-call services to patients/families as assigned
• Other duties as assigned

Required Qualifications:

• Current RN license, specific to the state(s) you are assigned to work
• Current CPR certification
• 1+ years of nursing experience
• Valid driver's license, reliable transportation and liability insurance

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $71,640 to $107,460 annually based on full-time employment. We comply with all minimum wage laws as applicable.
