Job summary

Temecula Valley Hospital (TVH), part of Southwest Healthcare, is seeking a Case Manager to perform assessments, determine care needs, and initiate discharge planning across settings. The role emphasizes timely evaluation of functional status, diagnosis, and cultural considerations.

Candidates must have two years in an acute care hospital, completion of an accredited RN program, and CA RN license; CCM/CPUR preferred; CPI within 90 days. TVH offers growth opportunities and competitive benefits.

Qualifications

Responsibilities

• Assess patients according to functional status, diagnosis, care setting, age, development level, and cultural orientation.
• Initiate and oversee discharge planning for each patient on admission as appropriate.
• Evaluate and document response to the Plan of Care and interventions; update Plan of Care as needed.

Case Manager

The Case Manager is responsible for performing assessments appropriate to functional status, diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner. The candidate will also identify and prioritize the need for further assessment based on the patient’s clinical presentation/diagnosis, care setting, desire for care and responses to any previous care. The Case Manager initiates discharge planning for each patient upon admission as appropriate. In addition, the Case Manager evaluates and documents the patient’s response to the Plan of Care and to interventions; revises and updates the Plan of Care as evidenced by documentation.

Qualifications

Education/Training/Experience

• Two (2) years experience in an acute care hospital
• Case Management/Discharge Planning/Utilization Review experience preferred
• Completion of an accredited Registered Nursing program
• Current California Registered Nurse license
• CCM and/or CPUR certification preferred
• CPI Certification within the first 90 days
