Position Title: Psychiatric Rehabilitation Program (PRP) Case Manager
General Description:
The PRP Case Manager is responsible for providing case management services to individuals recovering from mental illness. The Case Manager works with individuals, the service team, and other providers to ensure the services and resources are in place to meet the needs of the individual. Schedule: Full time, 40 hours weekly. This person reports to the Psychiatric Rehabilitation Program Director. FLSA: Non-Exempt.
Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:
A Bachelor's Degree plus one (1) year experience in mental health/psychiatric rehabilitation is required, or an AA degree with at least two (2) years experience in mental health/psychiatric rehabilitation may be considered. Must demonstrate the ability to work as part of a team and independently. Effective verbal and written communication skills are required. The ability to respect individual choice and the recovery process is essential. A good driving record is required.
