Central Utah Rehabilitation Hospital, part of Ernest Health, seeks a PRN Case Manager to join our compassionate care team in Provo, Utah. You will collaborate with physicians and a multidisciplinary team to plan and monitor patient care, while supporting discharge planning and post-discharge needs.

Qualifications include current clinical licensure is preferred and at least one year of recent relevant experience in an acute care setting.

• Actively involved in discharge planning process.
• Integrates the hospital’s mission and “Guiding Principles” into daily practice.

Qualifications

Required Skills:

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.
