Job summary

Helen M. Simpson Rehabilitation Hospital, in partnership with UPMC and Select Medical, seeks a Case Manager for our inpatient program at UPMC Osteopathic.

This per diem role coordinates care decisions, discharge planning, and cross-continuum care to improve outcomes and manage costs. Responsibilities include clinical interventions, discharge planning, care planning management, payer/referral management, and ensuring timely hospital-based services.

Qualifications

• Current licensure per state guidelines in a clinical or related discipline.
• Bachelor's or Master's degree in a health or human services discipline.
• CCM certification preferred.

Responsibilities

• Coordinate health care decisions to improve outcomes.
• Coordinate care across the continuum and discharge planning.
• Ensure cost-effective care within the designated time frame.
• Assure payments of hospital-based services meeting utilization criteria.
• Assess psychosocial needs and provide discharge planning interventions.

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Minimum Qualifications

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Preferred Qualifications

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.
