St. Luke’s University Health Network in Bucks County, PA seeks a discharge planning professional to evaluate psycho-social needs of rehabilitation patients and coordinate comprehensive discharge plans.
The role requires collaboration with patients, families, and an interdisciplinary team to ensure safe, timely transitions and appropriate post-discharge services. Responsibilities include ongoing assessments, documentation, and securing authorizations while addressing barriers to care.
MSW, RN (BSN preferred), OR Masters in a related field of physical, occupational, or speech therapy may be considered based upon experience and knowledge of care management and discharge planning.
Preference is for at least two to five years of clinical experience in healthcare preferred, preferably in a rehabilitation hospital.
