Job summary

Quality Moments Behavioral Health in Ohio is seeking a Senior Care Case Manager to provide therapeutic activities and companionship for individuals in skilled nursing facilities. This role may be filled as either part-time or full-time.

You will build meaningful relationships with aging adults, advocate for dignity, and support quality of life through person-centered care. A valid driver’s license and reliable transportation are preferred.

Qualifications

• Must be at least 21 years of age.
• Diploma or GED required.
• Desire to improve quality of life for aging adults.
• Must have experience working with Alzheimer patients.
• High level of commitment, dependability, and interpersonal skills.
• Prior experience in STNA, CNA, or long-term care is preferred.
• Prior experience in social work, counseling, human services, or related field is preferred.
• Valid driver’s license and reliable transportation with current auto insurance meeting state requirements.
• Previous EHR system use is helpful.
