Job summary

TemPositions Health Care is seeking a Part-time Case Manager for our Patchogue, NY office to provide professional intervention and critical services to clients.

Responsibilities include assessment, crisis intervention, and financial assistance, with a focus on advocacy, home visits, and documentation across multiple sites. The role supports a full range of social services and requires traveling within the area.

Qualifications

• BS degree in Human Services, Sociology, Psychology, or equivalent.
• One year experience in social services setting.
• Good organization and computer skills required.
• Able to conduct home/field visits and travel within the five boroughs is a must.
• Monday - Friday - Total 30 hours a week.

Responsibilities
