Aura Health seeks an LMSW Case Manager to deliver patient-centered care within our Healthcare & Medical Services team in Arlington, VA. You will assess psychosocial needs, develop tailored care plans, and advocate for patients through their care journey.

You will collaborate with physicians, nurses, and community partners to support safe discharges, improve health outcomes, and optimize workflows using EMR data and evidence-based practices.
