Job description

The Case Manager is responsible for providingsocial services to children, youth and families that are consistent with theMCH Model of Care and ensuring a safe, caring and therapeutic environment forthose served. This position requires the intellectual and emotionalmaturity to work with families who are experiencing difficulty in providingappropriate care for the children in their home and children who havebehavioral issues, developmental delays and mental health needs. This positionalso requires respect and compassionate understanding for these children andtheir families, and the ability to form effective working relationships withchildren, parents and other caregivers. CaseManagers must have a thorough understanding of and demonstrated competency inthe Trust-Based Relational Intervention (TBRI) and other evidence-basedprograms offered in the assigned area. The Case Manager must maintain a healthyvalue system, including moral and ethical behavior consistent with the agency'smission and core values.

DUTIES AND RESPONSIBILITIES
  • Identifyand assess eligible participants according to program criteria and obtainconsent for services for those accepted into the program, and make referrals toother community resources for those not accepted into the program.
  • Developand implement written plans of service based on the assessed needs of the childor family in accordance with program guidelines. Revise and update the written plans ofservice as needed to address the changing needs of those served and to complywith program guidelines.
  • Provideservices in compliance with program guidelines and as outlined in the plan ofservice.
  • Maintaina regular schedule of contacts and case management services on the assignedcaseload in accordance with program guidelines and the needs of each case. Goals and methods related to these contactsare to be established in consultation with the supervisor, parents/managingconservators and, when possible, with the children and youth served.
  • Coordinate,consult and collaborate with foster families and/or kinship caregivers andother service providers to provide needed services to children, youth andfamilies.
  • Providekinship caregiver home support and/or aftercare support, in compliance withT3C.
  • Maintainaccurate and timely documentation in the client information system and clientfile, including all relevant contacts, activities, incidents, appointments,service plans, reports, case notes, case logs, correspondence and otherrequired case documentation.
  • Keepsupervisor, parents/managing conservators, or other involved parties informedof incidents and other developments in the case as required by programguidelines, agency policies, contract provisions and applicable licensingstandards and accreditation standards.
  • Makerecommendations for continuation, closure or change in services.
  • Leadparent education groups or other non-clinical groups as assigned.
  • Promoteawareness of MCH programs and participate in program recruitment activities.
  • Maintaincompliance with agency policies/procedures and licensing and accreditationstandards.
  • Manageagency funds, resources and equipment according to agency policy andprocedures.
  • Workcollaboratively in a proactive and positive manner with other departments.
  • Representagency at community functions, activities and events.
  • Ensureall facilities and equipment are clean, organized and maintained.
WORKING CONDITIONS

This is aprofessional position that may require more hours than the normal 40-hour workweek.This position involves providing services in the client's home and extensivework in the community. Responsibilitiesinclude work in the evenings and on weekends. This position requires theability to serve in an on-call capacity and respond to emergencies after officehours. Out of town and overnight travel are also required. An automobile for intown and out of town travel is required. Mileage reimbursement is provided foruse of personal automobile for approved business purposes. A technology stipend isprovided to support the communication requirements of this position.

EDUCATION/LICENSURE

A Bachelor's Degreein Social Work or related field is required with a Master's Degreepreferred. Relevant experience inprograms and services offered by MCH is preferred.

TRAINING

Training hours mustbe completed annually to comply with licensing and accreditation standards, aswell as MCH policy. The employee's supervisor may require additional trainingto meet specific job requirements or enhance professional growth. Completion oftraining requirements is mandatory for continued employment.

OTHER

The Case Manager mustbe at least 21 years of age and have a valid Driver's License, an excellentdriving record that meets the standards set forth by the liability insurancecompany and Methodist Children's Home, no criminal history and acceptablereferences.
