Oaks Integrated Care Inc. is seeking qualified Case Managers for the Justice Involved Services (JIS) program in Burlington and Mercer Counties, New Jersey.

The role focuses on diverting nonviolent individuals with mental health concerns from jail to community services and coordinating care across mental health, criminal justice, and social services systems. Responsibilities include intensive community-based support, service linkage, and crisis intervention.

Qualifications

• MSW or Master’s degree with clinical training or a BA/BS in a related field with 1 year post-degree relevant experience.
• Valid driver’s license in good standing.

Responsibilities

• Provide intensive community-based services to seriously and persistently mentally ill consumers.
• Facilitate service linkage across mental health and other health systems.
• Provide direct service support: transportation, arranging treatment teams, filing applications, advocacy, appointment escorts, daily living/money management coaching.
• Maintain accurate clinical records electronically in a timely manner.
• Provide 24/7 on-call crisis intervention on a rotating basis, including evenings/weekends.
• Collaborate with the Clinical Team to coordinate care.
• Travel within Mercer, Camden and Burlington Counties as required.

Oaks Integrated Care is actively seeking qualified Case Managers for our Justice Involved Services (JIS) program located in Burlington and Mercer Counties. JIS works closely with local law enforcement to divert nonviolent mental health consumers from the jail system to appropriate mental health services in the community.

The Case Manager will have the ability assess and interpret information related to population; have knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; knowledge of available treatment resources and appropriate use; Population-specific communication skills.

Justice Involved Services (JIS) specialize in assisting persons with mental health concerns in connecting with appropriate and available resources. Our goal is to divert nonviolent persons with mental health from the criminal justice system to appropriate mental health services in the community. The JIS program increases coordination of resources between the mental health, criminal justice, and social services systems. Working together creates a bridge leading to positive outcomes for people who may otherwise be incarcerated.

Schedule: Mon-Fri 8:30p-5:00p with 30 minute unpaid lunch.

Responsibilities

• To provide intensive community-based services to seriously and persistently mentally ill consumers in the community
• To facilitate service linkage in the mental health and non-mental health, health system coordinating and integrating services from multiple providers
• To provide direct service support including: transportation, coordinating treatment team meeting; filing applications for services; aggressive advocacy; escort to appointment; daily living skill remediation and money management
• Responsible for maintaining a consumer clinical record(electronically) that accurately reflects all services provided on a timely basis and meets Agency and funding source standards
• Responsible for completing of Daily Activities in the Electronic Clinical Record
• To provide 24 hours/7 days a week on call crisis intervention on a rotating basis, including evenings and weekends
• To work collaboratively with the Clinical Team
• Driving and travel within within Mercer, Camden and Burlington Counties required.

Qualifications
