Frontier Behavioral Health is seeking a Case Manager III to deliver clinical case management for adults in long-term services, develop care plans, and assess needs to promote health and independence. You will supervise newer staff, educate on policy updates, and coordinate advocacy across agencies.
The role requires a Master’s or Bachelor's in related fields with several years of social service experience, plus travel between FBH worksites and strong communication skills.

The Case Manager III provides clinical case management services to adults receiving long-term term services and supports. Evaluate client strengths and needs, complete a comprehensive assessment, determine level of functioning, develop and implement service plans that provide a reliable and safe plan of care that enables clients to achieve a maximum level of health and independence and targets maintaining case stabilization, ensuring independence. Complete administrative duties and meet required program deadlines. Provide presentations and/or community education as determined. Coordinate with and perform advocacy with relevant parties and community agencies. Provide oversight to Case Manager II’s, specifically newly hired staff who are unfamiliar with the CARE system. Provide training to Case Management staff on changes to DSHS policy, management bulletins, and any correspondence altering processes received from ALTCEW.
Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire. Must have an unrestricted and valid driver’s license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work.

The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required. Daily use of a computer is required. May be required to lift 10 - 20 pounds. Blood‑borne Pathogen Category III - No predictable exposure to blood‑borne pathogens.
$29.26-$35.65 All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $29.26 per hour.
