What You’ll Do
- Facilitate admission intakes, treatment planning, discharge planning, and legal process assistance for individuals in crisis.
- Collaborate with the interdisciplinary treatment team to coordinate care and discharge planning for individuals, families, clinics, local providers, and other referral sources.
- Educate and inform individuals of consents for treatment and authorizations for use and disclosures.
- Participate in Interdisciplinary Treatment Team meetings.
- Develop and implement individualized person‑centered Treatment Plans, complete and review signed plans with participants.
- Gather collateral to assess individual treatment and discharge needs.
- Actively coordinate care and provide referrals to ensure services are synchronized with facility staff and external providers.
- Identify and address gaps in service delivery and recommend appropriate solutions for discharge needs.
- Engage, monitor, and communicate with individuals to maintain safety and meet their needs.
- Maintain clinic records, including documentation of activities and obtaining required signatures.
- Maintain working knowledge of the Virginia Involuntary Commitment process and the DBHDS/DMAS behavioral health system.
- Participate in individual and group supervision, ongoing education, and competency assessments.
- Complete all required training, attend monthly meetings, and maintain required competencies.
- Coordinate civil commitment process with stakeholders.
- Provide daily programming/groups.
- Perform all other duties as assigned.
Qualifications
- Bachelor’s degree in behavioral health or a related field.
- Knowledge of the Commonwealth of Virginia behavioral health system of care and available resources.
- Qualified Mental Health Professional (QMHP).
- Full vaccination required (documentation or exemption per company policy).
- State of Virginia Fingerprint Clearance.
- Pass VA DBHDS background check.
- Pass VA DSS Office of Background Investigations - Central Registry Search.
Preferred Qualifications
- Master’s degree in behavioral health or a related field.
- Licensed Mental Health Professional (LMHP).
- Experience working with children, youth, families, and adults with behavioral health issues.
- Knowledge of and experience with the Virginia involuntary treatment process.

Actual compensation is based on relevant experience, education, internal equity, and budget. $30.20 – $37.08 USD per hour.
