Job summary

University of Maryland Medical System seeks a care manager to deliver patient-centered care management through comprehensive assessment, care planning, and care coordination. You will support safe transitions across care settings and help navigate benefits and resources for patients and families.

Responsibilities include developing care plans, educating patients, documenting thoroughly, and collaborating with interdisciplinary teams to improve outcomes and reduce barriers to care.

Qualifications

• Associate degree in Nursing or Bachelor's degree in Physical Therapy.
• Current Maryland unrestricted RN or PT license.
• One year of healthcare, care coordination, case management, utilization management, or related experience.
• BSN or related healthcare field preferred.
• Three years of relevant healthcare or case management experience preferred.
• Active membership in a relevant professional organization recommended.
• Experience in acute care, managed care, ambulatory, or population health preferred.
• Specialty certification in case management, utilization management, discharge planning, or related specialty preferred.

• Conduct comprehensive assessments of patients' clinical, psychosocial, functional, financial, and support needs.
• Develop, implement, and monitor individualized care plans aligned with patient goals.
• Coordinate services and resources across the continuum of care for timely access.
• Facilitate safe and effective transitions with a coordinated discharge plan and follow-up.
• Educate and engage patients and caregivers to support informed decisions and self-management.
• Assist patients in navigating benefits, resources, and financial assistance programs.
• Maintain accurate, timely, and compliant documentation per regulatory requirements.
• Collaborate with interdisciplinary teams to improve care coordination and outcomes.
• Participate in quality improvement and professional development activities.
• Perform all other duties as assigned.

• Maintain accurate, timely, and compliant documentation in accordance with regulatory, accreditation, and organizational requirements.
• Collaborate with interdisciplinary teams to improve care coordination, patient outcomes, and operational effectiveness.
• Participate in quality improvement initiatives and professional development activities to maintain current knowledge and skills.
• Perform all other duties as assigned.

Education & Experience - Required

• Associate degree in Nursing or Bachelor' degree in Physical Therapy.
• Current Maryland unrestricted licensure as a Registered Nurse (RN) or Physical Therapist (PT).
• One (1) year of healthcare, care coordination, case management, utilization management, or related experience.

Education & Experience – Preferred

• Bachelor of Science in Nursing (BSN) or other related healthcare field.
• Three (3) years of relevant healthcare or case management experience.
• Active membership in a relevant professional organization (e.g., ACMA, CMSA, CCMC, ANA, NASW).
• Experience in an acute care, managed care, ambulatory, or population health environment.
• Specialty certification in case management, utilization management, discharge planning, or a related specialty.
