While this work is meaningful and mission-driven, it can at times be stressful, demanding, or unpredictable.

Employees are expected to exercise sound judgment, remain aware of their surroundings, follow established safety, communication, and de‑escalation procedures, participate in all required trainings, and promptly report incidents, threats, injuries, or unsafe conditions in accordance with ECS policies and procedures.

This position is a fully onsite, client‑facing role that requires in‑person interaction and direct service delivery to participants at ECS program locations. Remote work or work‑from‑home arrangements are not available for this position.

Primary Duties and Responsibilities
  • Perform intake assessment of assigned residents on the day of move‑in, including reviewing health status, psychosocial history, financial status, drug and alcohol use history, and housing history.
  • Outreach to each resident on the assigned caseload regularly and in response to incidents or documentation indicating a crisis. Direct, in‑person engagement is always the goal.
  • Work with each resident to develop and implement an individualized Service Plan based on goals and needs.
  • Provide referrals to on‑site or external providers for substance use, vocational training, legal services, immigration services, physical and mental health services, housing resources, and money management.
  • Assess each resident’s eligibility for benefits and entitlements and assist with applying for and enrolling in these programs.
  • Assist residents with budgeting, including voluntary money management plans.
  • Advocate with Property Management on behalf of residents and work to avoid eviction when possible.
  • Help resolve conflicts to promote a respectful, cooperative, self‑sufficient living environment.
  • Participate twice per week in Wellbeing Checks and perform Emergency Safety Checks as needed.
  • Develop and facilitate at least one weekly therapeutic or psycho‑educational group and assist with other program events such as Residents’ Council, BINGO, BBQs, holiday celebrations, community meetings, movies, and outings.
  • Maintain accurate, confidential documentation in resident files and complete all required paperwork within the required timeframe.
  • Assist residents with planned exits, providing comprehensive discharge planning and referrals to case management, housing, food, clothing, medical treatment, detox, and other services.
  • Help develop and implement new programs, groups, and activities with the Support Services Manager.
  • Provide back‑up coverage for other case managers as directed.
  • Share information and new resources with residents and staff.
  • Follow all applicable ECS policies and procedures.
  • Establish a positive working relationship with employees, volunteers, residents, and outside agencies.
  • Attend all assigned meetings.
  • Perform other duties as assigned.
Qualifications – Case Manager
  • High School Diploma + 4 years’ experience; or AA degree in a similar field + 3 years of experience; or CHW Certificate/CADC + 2–4 years’ experience; or BA/BS degree + 2–4 years’ experience; or MA/MS degree + 1 year experience; or equivalent combination.
  • For Senior Case Manager (at $30/hr): MA/MS + at least 2 years’ experience; or BA/BS + at least 4 years’ experience; or CHW Certificate/CADC + 4 years’ experience.
Additional Qualifications and Desirable Skills
  • Knowledge of local social services and community resources for underserved populations.
  • Patience, compassion, and empathy toward participants.
  • Excellent charting, record‑keeping, and professional writing skills.
