Job summary

Adjoin Catalysts seeks dedicated professionals to deliver person-centered support across living, work, and community activities in California. You will assess client needs, coordinate placements, and supervise direct support staff within a collaborative team environment.

Role requires 18+, a Bachelor's degree or 3 years related experience, strong interpersonal skills, and ability to manage multiple tasks with confidentiality.

Responsibilities

• Process referrals from appropriate referring agencies.
• Expedite the intake and assessment process.
• Manage, develop and evaluate direct support staff.
• Assess client data and conduct ecological studies.
• Develop placements for clients in work or living situations.
• Participate in ongoing training and policy updates.
• Prescribe, implement and supervise individual training and support programs.
• Project staffing needs and communicate with Regional Manager.
• Collaborate with agencies, families and clients toward service goals.
• Participate in hiring, training, evaluation, and separation of staff.
• Complete logs, reports and client evaluations.
• Review staff documentation for accuracy and completeness.
• Evaluate client and personnel needs and address issues appropriately.
• Provide direct service as needed and be on-call as required.
• Travel to job fairs, meetings, and employer sites as needed.

Responsibilities

• Process referrals of clients from appropriate referring agencies.
• Expedite the intake and assessment process.
• Effectively manage, develop and evaluate direct support staff.
• Assess client collateral data, information from intake interview, and conduct ecological studies of client's abilities and potential living and work environments.
• Develop placements for clients in work and/or living situations that best match the individual's strengths and availability.
• Participate in ongoing training and development, taking responsibility for keeping updated on changes in policies, procedures and protocol.
• Prescribe, implement, and supervise individual training and support programs.
• Project staffing needs and communicate those needs to the Regional Manager and/or his/her designee.
• Collaborate with and develop positive communications and relationships with individuals in cooperative agencies, employers, involved members of families and clients towards the achievement of service goals.
• Participate in the hiring, training, evaluation, and separation of direct support staff.
• Complete in a timely manner all logs, reports and client evaluations as required.
• Review and assure all direct service staff’s documentation is accurate and complete.
• Evaluate and respond appropriately to client and personnel needs and issues, recognizing and taking appropriate action where needed and elevating concerns through chain of command as appropriate.
• Provide direct service as needed and required.
• Be part of the on-call rotation as needed and required.
• Travel to job fairs, client meetings, employer sites, and other locations as required, ensuring safety and professionalism while driving.
• Advocate and perform duties with a person-centered methodology.
• Other duties as required.

Requirements

• 18 years of age or older.
• Bachelor's degree or three years equivalent experience and/or educational equivalent in working with individuals with disabilities.
• Since this position requires a great deal of interaction with people, well-developed interpersonal skills are critical.
• Demonstrated computer literacy (MS Office) and capability to maintain well-organized records and files.
• Ability to follow detailed, multi-step instructions is critical to this position.
• Ability to multi-task, work under pressure and manage time is also an essential component of this position.
• Demonstrated ability to effectively navigate and maintain professional boundaries as custodian of confidential information.
• Certification in an approved First-Aid and CPR class; or successfully complete an approved First-Aid and CPR class within 45 days of the date of hire.
• Insured, registered vehicle in safe working condition, and proof of current CA driver’s license.

POSITION REQUIREMENTS

• Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.
• Ability to travel in and throughout the state on a quarterly basis to attend meetings, trainings, etc.
• Successful completion of pre- employment drug screen and background check.
• Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
• Acceptable DMV record.
• Adjoin’s client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.
